EOPS Progress Report

It’s progress report time! Remember, you must complete and submit a progress report to be eligible for the EOPS textbook service and grant.  Eligible EOPS students will receive a grant in June 2023 and textbook services for Summer 2023 and Fall 2023!  To be eligible, students must follow the EOPS Mutual Responsibility Agreement, which includes submitting a progress report. 

The progress report helps us support your academic success.  It is also an opportunity for you to develop the habit of speaking with your instructors about your progress in your classes and develop relationships with them. For the Spring 2023 semester, the progress report is due Sunday, April 16 at 11:59 PM. Please be aware that late progress reports will not be accepted. 

We encourage you to submit your progress report by the due date even if you have not received feedback from your instructor(s).  Simply copy/paste your e-mail to the instructor into the appropriate box on the progress report form.

You only need to complete the online progress report once, even if you participate in multiple special programs.  However, you may make multiple submissions if you are still waiting for feedback from one of your instructors. To submit your progress report, you will be asked to complete two steps for each of your classes: 

  1. Provide your current grade.  Look it up on Canvas or contact your instructor as part of step two.  If you don’t have a grade yet, please indicate N/A. 

  1. Provide feedback from your instructor.  Contact your instructors through Canvas or e-mail to obtain their feedback on how you are doing in their class.  Then, copy/paste their response(s) into the online progress report.  If your instructor(s) does not respond, please copy/paste your e-mail to your instructor into the appropriate box.  

Here is a sample message to your instructor: 

Dear Professor, 

My name is (write your name) and my student ID is (write your student ID). I am currently enrolled in the following class (write the class you are enrolled in). As an EOPS student, I would like to know my current grade (if available) and feedback on how I am doing in your class.  Any comments or recommendations you may have on how I can improve my grade in your class would be helpful.   

Please respond by (insert date)

  • If you are enrolled in a late start class, please answer the appropriate questions. 

  • Should any of your instructors have a prolonged absence or if you have any problems completing your progress report, let us know so that we can help you find an alternative solution.
