SLO Forms
The following are the forms that are needed to submit SLOs and assessment cycle reports. Course and Program-Level SLOs are reviewed by the Valley College Curriculum Committee and then forwarded to the Senate. All other SLOs are reviewed by the SLO Steering Committee.
Course-Level SLO form - submitted to Erline Ewing in Academic Affairs along with a Discipline Approval Form
Course-Level Assessment Report - submitted to SLO Coordinator, Rebecca Stein
