To petition for the modification of a department structure by either adding or removing disciplines, complete the appropriate form below and email it to the PEPC Chair.  A signed department approval form with the signatures of a majority of the members of the department should be scanned and submitted electronically at the same time.  If you need help with this process, contact Tyler Prante.

Department Name Change

To petition for a change in the name of a department, complete the form below and email it to the PEPC Chair.  A signed department approval form with the signatures of a majority of the members of the department should be scanned and submitted electronically at the same time.  If you need help with this process, contact Tyler Prante. 

Note: This form is for a name change only. If modifications to the department structure are being requested, the Department Modification form should be used instead.
