REGISTER

REGISTRATION PROCEDURE FOR CAMP

Registration for Summer Camp will open on Monday March 25, 2013 and will remain open until all spots in camp are filled or the Wednesday prior to the start week of camp.  You may pick up forms at the Community Services Office or download them here. PLEASE NOTE: YOU MUST TURN IN COMPLETED FORMS AT THE COMMUNITY SERVICES OFFICE.  YOU MAY MAIL OR FAX YOUR FORMS TO US IF ONLY YOU ARE REGISTERING ON LINE FOR ANY OVERNIGHT ADVENTURE CAMP OR ANY SUPERSAVER PACKAGE. YOU MUST REGISTER IN PERSON FOR ALL OTHER PROGRAMS.

IMPORTANT NOTE FOR SUPERSAVER REGISTRATIONS: After registering on-line you MUST CALL us to let us know which specialty camp your child will attend each week. If you do not call us we will hold space ONLY in the regular day camp program.

Regular registration hours are from 8:00 am to 5:30 pm, Monday through Thursday at the Community Services Office and on Fridays from 8:00 am to 12:00 noon. The Community Services Office is located in the Field House on Ethel Avenue across from the pool. Click here for a map of campus. PLEASE NOTE THAT WHEN COMING TO OUR OFFICE TO REGISTER IN PERSON YOU SHOULD PARK IN PARKING LOT L LOCATED EAST OF ETHEL AVENUE. DO NOT PARK ON ETHEL AVENUE BETWEEN BURBANK BLVD. AND HATTERAS STREET. Ethel Avenue is reserved parking for faculty and staff only. Parking on Ethel Avenue may result in a parking violation citation.

Credit card registration is available 24 hours/day by fax machine (818) 947-2930. You must complete and submit all camp registration forms before paying by phone or fax. We accept American Express, Discover, MasterCard and Visa credit cards.

Fees include a camp T-shirt (one per summer to wear on camp activity days). Extra camp T- shirts may be purchased for $7.00 at the Community Services Office. ALL forms must be filled out completely before a child may be placed in the camp or assigned to a group.

PLEASE NOTE FOR ON-LINE REGISTRATION:
After you have clicked on one of the links above a new window will open in your browser. Ensure that the program you want to enroll in is correct and click on the "add to cart" button. You will then be prompted to sign-in or create a new account. If you have already registered with the Community Services Department for ANY class since January 2010 and know your user name and password you will be able to sign-in. If you have not registered since January, 2010 you will need to create a new account.  After you have signed in you will be asked for your credit card information. We accept American Express, Discover, MasterCard and Visa.

IMPORTANT NOTE: REGISTRATION IS NOT COMPLETE AND NO SPACE WILL BE RESERVED UNTIL ALL FORMS ARE COMPLETE AND ON FILE AT THE COMMUNITY SERVICES OFFICE. YOU MUST COMPLETE NEW FORMS EACH YEAR.
By registering on line you agree that you are aware that even though payment has been made in full we will NOT hold a space for your child until we have a completed and signed Parent Card and Permission To Treat/Field Trip Waiver Form on file. We will only hold space in this program for children who have a parent card on file. In the event that you do not complete the parent card prior to all spaces in the camp being filled you will receive a refund of fees paid LESS $35.00 processing fee.
You may obtain a parent card and a permission to treat/field trip waiver form by calling us at 818.947.2577 x4172 or by downloading and printing them at this site. Once you have completed the forms you may bring them to our office in the Field House or you may send them to us at:

PLEASE NOTE THAT THE COMMUNITY SERVICES DEPARTMENT IS NOT RESPONSIBLE FOR LOST OR MISDIRECTED MAIL AND WE STRONGLY RECOMMEND THAT YOU BRING THE FORMS IN PERSON TO OUR OFFICE.
